Osteoarthritis (OA) of the knee is a degenerative condition that affects approximately 10% to 15 % of people older than 50 years and is by far the most common cause of knee pain in those older than 50 years. Pain tends to be progressive and is often debilitating, thus leading to a significant decline in the quality of life in these individuals. While there are many nonoperative treatment options and joint-sparing procedures available for the management of early OA, the gold standard for treatment of end-stage OA is total knee replacement surgery (TKRS). Currently, there are approximately 200,000 total knee replacements being performed annually in this country and more than 500,000 worldwide
Arthroscopy of the knee is by far the simplest operative treatment that can be chosen for management of the arthritic knee. This is an outpatient procedure that can generally be completed in less than 30 minutes, and the complete recovery time is often less than 2 weeks. The procedure is done simply to “clean out” the knee by removing any loose chondral flaps, debris in the joint, or torn or frayed meniscus. Arthroscopy cannot be used to replace any of the diseased cartilage and exists only to delay the need for more invasive intervention. Several well-designed studies have indicated that there is no benefit to arthroscopy in the degenerative knee.
Unicompartmental knee replacement surgery is another treatment option for those with single compartment disease. It is used most commonly in those with a varus deformity that has led to isolated degeneration of the medial compartment. This procedure entails a resurfacing, or replacement, of the degenerative compartment by removing the native articular cartilage from the medial femoral condyle and medial tibial plateau and replacing it with prosthetic components (Fig. 18.14). The primary advantage of UKA over HTO is that it allows for a much faster recovery and permits immediate weight-bearing in the postoperative period
The gold standard for the treatment of end-stage OA or arthritis of the knee is total knee replacement. This is an extremely common operation, and it has a proven track record for both survival and pain relief. As mentioned earlier, there are several surgical alternatives, but none has been able to approach the long-term results of Total knee replacement surgery ( TKA )
